Job summary

KabaFusion in Carmel, IN seeks a licensed Pharmacist to provide safe, patient-centered pharmacy services in the home infusion setting. You will assess therapy, dispense medications, document progress, and coordinate patient care with clinicians.

The role requires 2+ years in acute care or 1 year in home infusion, IV admixture experience, and strong communication. Excellent organizational skills and supervision of staff are essential for success.

Qualifications

• Bachelor of Science in Pharmacy required
• Current Indiana pharmacist license required
• Minimum 2 years recent experience in an acute-care setting or 1 year in home infusion
• Experience with intravenous admixture services and intravenous therapy preferred
• Ability to supervise technical and support staff and coordinate care with physicians, nurses and other healthcare professionals

Responsibilities

• Provide safe and appropriate pharmacy services in home infusion setting per policies and standards
• Assess suitability of therapy and design patient-specific plans of care
• Dispense medications, supplies and equipment and document patient progress
• Maintain patient medication profiles and accurate electronic documentation
• Submit electronic claims for medications and document communications with care team
• Oversee pharmacy support personnel and delegate non-clinical tasks as needed
• Participate in patient care conferences and on-call activities as appropriate
