Job summary

A-line Staffing Solutions is recruiting a Pharmacist for an on-site home infusion pharmacy in Oklahoma City, OK. The Pharmacist will evaluate and monitor drug therapy and ensure compliance with regulations while collaborating with nurses, customer service, and distribution teams.

Responsibilities include reviewing patient orders, planning and monitoring therapy, and conducting assessments for drug-related problems. Acute-care or home infusion experience is required.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in Oklahoma.
• Minimum of two (2) years recent experience in an acute-care setting or one (1) year in home infusion therapy.
• Experience with compounding infused drugs.

Responsibilities

• Evaluates and monitors drug therapy in home care patients.
• Coordinates referrals with nurses, customer service, and distribution teams.
• Reviews and documents all new patient orders.
• Assists healthcare providers with planning, implementing, and monitoring drug therapy.
• Conducts assessments to identify drug-related problems and evaluates therapy outcomes through laboratory and clinical data.
