Amerita, Inc. is seeking a Pharmacist to join our Pharmacy team in our Oklahoma branch. The role involves pharmaceutical care for home infusion patients, reporting to the Pharmacy Manager, and coordinating with nurses and physicians.

The ideal candidate has a PharmD and active pharmacist license, with 1–2 years in acute care or home infusion, and strong communication and organizational skills. On-call and rotating schedule may apply.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist license in state(s) of practice.
• Minimum 2 years acute-care or 1 year home infusion experience.

Responsibilities

• Evaluate and monitor drug therapy for home care patients with healthcare team.
• Adhere to all laws, policies and procedures governing pharmacy practice.
• Coordinate referrals and admissions with nurses, CS and distribution.
• Review and verify new patient orders and document accordingly.
• Assist physicians with in-home patient care and therapeutic objectives.
• Monitor outcomes of drug therapy using lab and clinical data.
• Provide drug information to patients, families, and healthcare professionals.
