Medix is recruiting a Home Infusion Pharmacist for a growing specialty home infusion program in West Chester, OH. This on-site role welcomes Pharmacists from retail, hospital, specialty, infusion, or closed-door settings who want a patient-centered clinical focus.

You will review orders, compound sterile IV meds, verify medications, and collaborate with a multidisciplinary team to optimize patient outcomes in a home infusion setting.

Qualifications

• Active Ohio Pharmacist license in good standing.
• PharmD or Bachelor of Science in Pharmacy from an accredited program.
• Sterile compounding or clean room experience preferred.
• Hospital, specialty, infusion, retail, or closed-door pharmacy experience welcomed.

Responsibilities

• Review and verify physician orders for home infusion therapies.
• Compound or oversee preparation of sterile IV medications per USP standards.
• Verify accuracy of medications, labeling and supplies before home delivery.
• Collaborate with physicians, nurses, and case managers to ensure safe patient care.
• Monitor patient therapy, lab results, and provide clinical recommendations.
• Educate patients and caregivers on medication administration, storage, and safety.
• Supervise pharmacy technicians and daily operations.
• Maintain compliance with state, federal, and accreditation standards.

A rapidly growing specialty pharmacy is seeking a Home Infusion Pharmacist to join its expanding team in West Chester, OH. This is an excellent opportunity for pharmacists from retail, hospital, specialty, infusion, or closed-door pharmacy backgrounds who are interested in working in a collaborative environment focused on improving patient outcomes through home infusion therapy.

Location: West Chester, OH

Pay: $58.00-$65.00/hour

Schedule: Full-Time | Monday-Friday | 8:30 AM-5:00 PM | Onsite

What You'll Do:

• Review and verify physician orders for home infusion therapies, including IV antibiotics, TPN, specialty biologics, and other infusion medications.
• Compound or oversee the preparation of sterile IV medications in accordance with USP <797> and USP <800> standards.
• Verify the accuracy of medications, labeling, and supplies before home delivery.
• Collaborate with physicians, nurses, and case managers to ensure safe, effective patient care.
• Monitor patient therapy, laboratory results, and make clinical recommendations when appropriate.
• Educate patients and caregivers on medication administration, storage, and safety.
• Supervise pharmacy technicians and support daily pharmacy operations.
• Maintain compliance with all state, federal, and accreditation standards.

• Strong clinical knowledge and commitment to medication safety.
