Schedule

Monday - Friday 8:30am - 5:00pm

Responsibilities
  • Evaluates and monitor drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Adheres to all appropriate standards of accreditation and participate in quality assurance activities.
  • Coordinates and facilitate new referrals and admissions through collaboration with nurses, customer service and distribution.
  • Reviews, receives and verifies all new patient orders and documents on the respective order form.
  • Assists the physician and other involved healthcare professionals with:
    • Planning and implementing patient care in the home;
    • Setting therapeutic objectives for drug therapy;
    • Establishing parameters for monitoring the safety and efficacy of drug therapy; and
    • Preparing regular progress reports.
  • Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug‑related problems or needs.
  • Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in‑services, seminars and conferences.
  • Compounds sterile mixtures and non‑mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.
  • Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards.
  • Documents all activities on appropriate logs.
  • Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules.
Qualifications
  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
  • Registered Pharmacist License in state(s) of practice
  • Minimum of two (2) years of recent experience in an acute‑care setting or one (1) year of recent experience in home infusion therapy
  • Experience with compounding infused drugs
  • Proven record of evaluating and monitoring drug therapies for home care patients
  • Good organizational and time‑management skills
  • Participation and membership in professional societies and organizations is desirable
  • Excellent verbal and written communication skills
  • Flexible schedule with the ability to work on‑call evenings and weekends on an as‑scheduled basis
  • Percentage of Travel: 0-25% - Minimal travel may be required

To perform this role will required constant sitting and typing on a keyboard with fingers, and occasionally standing, and walking. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs.

Salary Range

USD $65.00 - $70.00 / Hour
