Qualifications

• Active and unrestricted RN license in state of practice.
• Infusion nursing experience preferred or required.
• Current CPR certification and valid driver's license.

Responsibilities

• Initiates and implements basic nursing plans of care in home or alternate infusion sites.
• Administers medications, educates patients and families, and coordinates with physicians.
• Documents clinical information in real time and communicates with the care team.
• Performs discharge planning coordination and forwards care summaries as needed.
