Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Plymouth, MA and surrounding areas. 20–25 hours per week, generally Monday–Friday 8:30am–5pm with flexibility. Drive your own registered vehicle (mileage reimbursement) and maintain an active Massacheuttes RN license in good standing.

The role requires direct patient care, care coordination, and education for home infusion therapy, aligned with care matrices and best practices in the home or alternate site setting.

Qualifications

• Current RN license by the state’s Board of Nursing, in good standing (Massachusetts).
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred.
• Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy and provide instruction as needed.
• Evaluate home environment and identify interventions to render it safe for prescribed care.
• Develop plan of care with the team and evaluate response to therapy.
• Manage schedule via company platform and communicate scheduling changes.
• Coordinate patient care via approved company platform and participate in nurse on-call rotation.
• Document all encounters per Soleo policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
