Job summary

Responsibilities

• Coordinate transition of patients to home infusion service and assess plan of care
• Teach patients, caregivers and home health nurses to administer infusion therapy
• Collaborate with the health care team to ensure timely progression across the continuum of care
• Maintain contact with patients and families via assessments by phone
• Support quality improvement by evaluating variances to protocols and participate in quality projects
