Job summary

The University of Vermont Medical Center is seeking a Home Infusion Nurse Coordinator (In Patient Home) to ensure smooth transition of patients to home infusion, educate patients and caregivers, and coordinate with the care team to optimize outcomes across the continuum of care.

Responsibilities include overseeing infusion procedures, vascular access, safety recommendations to the MD, and maintaining quality data while supporting quality improvement initiatives within the home infusion program.

Qualifications

• RN license required.
• 2–5 years of clinical experience such as case management or home/public health nursing.
• Infusion and vascular access experience preferred.
• BSN pursued as per institutional initiative.

Responsibilities

• Coordinate transition of patients to home infusion service.
• Teach patients and caregivers to administer infusion therapy at home.
• Coordinate with health care team to ensure progression across continuum of care.
• Maintain contact with patient and health care professionals via weekly or as-needed assessments.
• Support quality improvement and maintain data systems and inventories.
