Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or alternate infusion suite under a physician's plan of care.

The nurse works under structured supervision to ensure patient safety and continuity of care. Key duties include initiating nursing plans, assessing patient status at visits, mentoring Infusion Nurse I staff, educating patients and families, and documenting care for billing and compliance.

Competenze

• Must hold an active RN license.
• Experience in home infusion patient care preferred.
• Ability to educate patients and families on therapy plans.

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit, reassessing and clarifying physician orders as needed.
• Provides training and mentorship to Infusion Nurse I's as needed to ensure patient safety and compliance.
• Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs.
• Effectively and timely communicates with Option Care's Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care.
• Coordinates discharge planning and prepares discharge summaries with patient instructions and progress and goals when transferring care.
• Demonstrates compliance with agency operations, Option Care's policies and procedures, professional standards, regulations, and accreditation standards.
• Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders and document care.
• Responds promptly to patient requests and initiates emergency procedures as necessary.
• Accepts accountability for own practice and adheres to HIPAA regulations.
• Observes confidentiality of patient and proprietary Option Care information and explains services and charges to patients.
• Attends and completes required training modules and continuing education to maintain competency and licensure.
