Option Care Health is seeking a Clinical Pharmacist to provide safe and effective pharmacy services for home care patients in a dynamic, patient-centered setting. You will collaborate with nursing staff, monitor progress toward goals and educate patients, caregivers, and staff on appropriate medication use.

The role is entry level into clinical pharmacy in alternate sites, requiring a licensed pharmacist with knowledge of regulations, quality standards, and safety practices.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy
• One of the following options for experience (sterile IV admixture, non-sterile compounding/immunizations/MTM, ASHP residency, APPE, or equivalent)

Responsabilités

• Determines the suitability of individual patients for home care.
• Designs, recommends, monitors and evaluates patient-specific pharmacy care plans.
• Provides medication-use education to staff, patients and caregivers.
• Ensures continuity of pharmaceutical care to and from home care settings.
• Prepares and dispenses medications following policy and safety standards.
