Option Care Health is seeking a Clinical Pharmacist to provide safe, compliant pharmacy services in the home care setting. This entry-level role coordinates with Nursing, educates patients and caregivers, and supports care continuity across settings.

You will apply pharmacotherapy knowledge, pharmacokinetics concepts, and CEU opportunities to promote patient safety and effective medication use within regulatory standards.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy
• One of: sterile IV experience, non-sterile compounding, MTM, or residency/advanced practice training
• Strong customer service and communication skills

Responsibilities

• Determines patient suitability for home care and collects info.
• Designs and monitors patient-specific pharmacy care plans.
• Educates patients, caregivers and staff on medication use.
• Ensures safe dispensing and regulatory compliance.
