Responsibilities

• Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
• Supervise other home care staff as necessary.

Qualifications

• Must possess an active RN license in both VT and NH.
• ASN required, BSN or MSN preferred.
• Nights, Weekend, Holiday and on-call rotation required.

Education

• Required: Associates or better in Nursing.
• Preferred: Bachelors or better in Nursing.

Experience

• Required: 2 years nursing
