About the Role:

The Home Hospice Nursing Assistant (CNA) in Manitowoc County plays a vital role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes. This position focuses on supporting patients' comfort and dignity by assisting with daily living activities, monitoring health status, and offering emotional support to both patients and their families. The CNA works closely with the interdisciplinary hospice team to ensure that care plans are effectively implemented and adjusted according to patient needs. Minimum Qualifications:
  • Current and valid Certified Nursing Assistant (CNA) certification in the state of Wisconsin.
  • High school diploma or equivalent.

  • Strong communication skills and the ability to work independently in a home environment.
  • Valid driver’s license and reliable transportation to travel within Manitowoc County.
