Job summary

Qualifications

• Current and valid Certified Nursing Assistant (CNA) certification in the state of Wisconsin.
• High school diploma or equivalent.
• Demonstrated experience in providing personal care to patients, preferably in a home health or hospice setting.
• Strong communication skills and the ability to work independently in a home environment.
• Valid driver's license and reliable transportation to travel within Manitowoc County.
• Ability to work four days per week with a rotating weekend schedule.

Responsibilities

• Provide direct personal care to hospice patients, including bathing, grooming, dressing, and mobility assistance.
• Monitor and document patients' physical and emotional conditions, reporting any changes promptly to the nursing staff.
• Assist with basic medical tasks such as taking vital signs and supporting medication administration under supervision.
• Offer compassionate emotional support and companionship to patients and their families, helping to alleviate anxiety and promote comfort.
• Collaborate with the hospice care team to implement individualized care plans and participate in care coordination activities.
• Maintain a safe, clean, and comfortable environment in the patient's home, adhering to infection control and safety protocols.
• Educate family members on basic care techniques and provide guidance on how to support their loved ones effectively.
