Job summary

Safe Life Home Health Care is seeking a PRN Speech Therapist to provide therapy services in Algonquin, IL. The clinician will evaluate and treat patients per plan of care, maintain documentation, and coordinate with the care team and physicians. Travel to patients' homes is required, and a valid IL license, TB test, CPR, and insurance are mandatory.

The role emphasizes timely evaluations, family education, and collaboration with the agency to ensure safe, effective home-based therapy.

Qualifications

• Must be a licensed Speech Therapist by the state of Illinois.
• At least one year of Home Health Care clinical experience.
• car with valid insurance coverage required.
• Maintain annual TB test and CPR certification.

Responsibilities

• Assist physician in evaluating patients’ function and setting treatment goals.
• Perform initial evaluations within 48 hours of referral and follow-up evaluations every 30 days.
• Prepare clinical and progress notes and summaries.
• Update plan of care and communicate changes to the physician.
• Advise and consult with patient’s family and other health care staff.
• Coordinate care with agency DON and staff.
• Observe and report changes in patient condition or schedule.
• Instruct patient/family on equipment use and home therapy programs.
• Communicate with patients, caregivers, and agency staff.
• Provide quality care and address issues with DON/Administrator.
• Ensure timely completion of all tasks.

Position Overview

Provides Speech Therapy services to patients as directed by plan of care in line with the philosophy, objectives, and policies of the agency. Maintains responsibility for the implementation of follow through of ST procedures.

Responsibilities

• Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.
• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate coordinating plan of care with the physician as needed.
• Prepares clinical and progress notes and required summaries, as appropriate.
• Revises plan of treatment and contacts the physician for change in plan of care.
• Advises and consults with patient’s family and other health care staff.
• Coordinates care with agency DON and other staff as appropriate.
• Observes, records, and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.
• Instructs patient and/or family in the use of equipment safe patient management and home therapy programs as needed.
• Communicates effectively with patients, care givers, and families as well as agency staff.
• Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
• Assures that all tasks are performed in a timely manner.

Qualification

• Must be a licensed Speech Therapist by the state of Illinois.
• Must have at least one-year Home Health Care clinical experience.
• Car with required proof of insurance coverage.
• Maintains annual TB test, CPR certification and other required.
• Able to read and interpret instructions related to patient care.
• Able to effectively perform and prioritize multiple functions or task as needed.
• Must be able to travel to prospective patient’s place of residence.
