Job summary

Safe Life Home Health Care, LLC is seeking a licensed Physical Therapist to provide home-based therapy across LaSalle County, IL and surrounding areas. The role includes evaluating patients, developing plans of care, and supervising PT assistants in accordance with agency policies.

• At least one year of Home Health Care clinical experience.
• Must have a car with proof of insurance.
• Maintains annual TB test, CPR certification and other required.

Responsibilities

• Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.
• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days.
• Prepares clinical and progress notes and required summaries.
• Revises plan of treatment and contacts the physician for change in plan of care.
• Advises and consults with patient’s family and other health care staff.
• Coordinates care with agency DON and other staff as appropriate.
• Observes, records, and reports changes in patient condition and updates the agency on status or schedule.
• Supervises the Physical Therapy Assistant.
• Instructs patient and/or family in the use of equipment and home therapy programs.
• Communicates effectively with patients, care givers, families and agency staff.
• Provides quality care and reports issues to DON/Administrator.
• Ensures timely completion of tasks.
• Checks in on PT Assistants every 4-6 patient visits.

Position Overview

Provides Physical Therapy services to patients as directed by plan of care in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and maintains responsibility for the implementation of follow through of PT procedures.

• Observes, records, and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.
• Supervises the Physical Therapy Assistant.
• Instructs patient and/or family in the use of equipment safe patient management and home therapy programs as needed.
• Communicates effectively with patients, care givers, and families as well as agency staff.
• Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
• Assures that all tasks are performed in a timely manner.
• Checks in on Physical Therapy Assistants every 4-6 patient visits.

Qualification

• Must be a licensed Physical Therapist by the state of Illinois.
• Must have at least one-year Home Health Care clinical experience.
• Car with required proof of insurance coverage.
• Maintains annual TB test, CPR certification and other required.
• Able to read and interpret instructions related to patient care.
• Able to effectively perform and prioritize multiple functions or task as needed.
• Must be able to travel to prospective patient’s place of residence.
