Job summary

Safe Life Home Health Care is seeking a licensed Physical Therapist to provide in-home PT services in the Chicago area under a PRN schedule. You will evaluate patients, develop plans of care, supervise assistants, and coach families on home therapy protocols.

The role requires travel to patients' residences, strong communication with caregivers, and adherence to state licensure, CHAP accreditation, and Medicare standards. A reliable vehicle and insurance are essential.

Qualifications

• Must be a licensed Physical Therapist in Illinois.
• At least one year of Home Health Care clinical experience.
• Access to a car with valid insurance is required.
• Annual TB test and CPR certification must be maintained.
• Ability to read and interpret patient care instructions.
• Ability to travel to patients' residences as needed.

Responsibilities

• Assist physicians in evaluating patient function and setting treatment goals.
• Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days.
• Prepare progress notes and summaries; update plan of care as needed.
• Supervise Physical Therapy Assistants and coordinate care with the physician.
• Educate patients and families on home therapy exercises and equipment use.
• Communicate with patients, caregivers, and agency staff effectively.
• Document status changes and schedule changes promptly.
• Check in on PTAs every 4-6 patient visits.
• Collaborate with DON and other staff to ensure quality care.
• Maintain timely completion of tasks and follow through on plans.

Position Overview

Provides Physical Therapy services to patients as directed by plan of care in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and maintains responsibility for the implementation of follow through of PT procedures.

Responsibilities

• Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.
• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate coordinating plan of care with the physician as needed.
• Prepares clinical and progress notes and required summaries, as appropriate.
• Revises plan of treatment and contacts the physician for change in plan of care.
• Advises and consults with patient’s family and other health care staff.
• Coordinates care with agency DON and other staff as appropriate.
• Observes, records, and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.
• Supervises the Physical Therapy Assistant.
• Instructs patient and/or family in the use of equipment safe patient management and home therapy programs as needed.
• Communicates effectively with patients, care givers, and families as well as agency staff.
• Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
• Assures that all tasks are performed in a timely manner.
• Checks in on Physical Therapy Assistants every 4-6 patient visits.

Qualification

• Must be a licensed Physical Therapist by the state of Illinois.
• Must have at least one-year Home Health Care clinical experience.
• Car with required proof of insurance coverage.
• Maintains annual TB test, CPR certification and other required.
• Able to read and interpret instructions related to patient care.
• Able to effectively perform and prioritize multiple functions or task as needed.
• Must be able to travel to prospective patient’s place of residence.
