Job summary

HCR Home Care in Onondaga County, NY seeks a Therapy Field Staff Supervisor (TFSS) to oversee home health services and coordinate clinical care across the patient care team.

The role requires a NYS therapy license, 3 years healthcare experience with at least 1 year in home health, and supervisory capabilities; you will ensure regulatory compliance, review plans of care, and support field clinicians.

Qualifications

• Bachelor of Science in Physical, Occupational or Speech therapy.
• Current, valid Therapy license (PT/OT/ST) issued by NYS.
• Three (3) years in health care with at least 1 year in home health care.
• Supervisory experience preferred.
• Knowledge of Medicare/Medicaid guidelines governing home health agencies.
• Organized, detail-oriented with strong communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Act as liaison to coordinate ongoing patient care and ensure appropriate plans are in place.
• Review Physician orders and Plan of Care (POC)/OASIS to reflect patient needs.
• Review ROC and discharge data; update orders as needed.
• Ensure compliance with HCR protocols and regulatory requirements.
• Provide support to field clinicians and coordinate care activities.
• Enter orders, update patient calendars, and communicate with Clinical Manager as needed.
• Participate in case conferences and multidisciplinary reviews.
