Job summary

Circadia Health is seeking a Patient Care Coordinator to visit homes in Orange County, install remote monitoring devices, take vitals, and arrange initial telehealth with our Nurse Practitioners. You will travel to patient homes across Victorville, Apple Valley, and surrounding areas, contributing directly to proactive care in patients' homes.

The role emphasizes independent work, patient education, and collaboration with care teams, with a weekly office visit for supplies and coordination.

Qualifications

• CNA or HHA certification required with demonstrated vital signs experience.
• Comfortable mounting and installing equipment with hand tools.
• Valid driver's license and reliable transportation for daily travel.
• Independent, patient-focused, and good communication with seniors.
• Familiarity with HIPAA and patient privacy standards.
• Willingness to pass a background check; bilingual Spanish a plus.

Responsibilities

• Take and record patient vitals during home visits and transmit readings.
• Oversee installation of Circadia devices at patient locations.
• Facilitate initial telehealth visits with Nurse Practitioners.
• Educate patients on device use and telehealth setup.
• Maintain device maintenance, IT setup, and equipment assembly.
• Coordinate with internal teams throughout the customer lifecycle.
• Audit asset list and assist supply chain planning for installs.
