Job summary

Avera Health is seeking a Speech-Language Pathologist to evaluate patients, develop care plans and provide therapies addressing speech, language, cognition, voice and dysphagia in the home setting. You will work with a multidisciplinary team to meet patient needs.

You will plan and deliver individualized therapy, document progress, and communicate effectively with patients, families, and the care team. A sign-on bonus and loan repayment options may be available.

Qualifications

• CCC-SLP certification or ASHA supervised fellowship in progress and certification within one year of employment.
• Active state license to practice as Speech-Language Pathologist.
• Hospital, SNF, outpatient or home health experience preferred.

Responsibilities

• Evaluates assigned patients using formal and informal assessments to determine intervention needs.
• Develops and implements individualized treatment programs for speech pathology.
• Sets and adjusts goals based on ongoing evaluations and coordinates with care plans.
• Delivers direct therapy using various settings and strategies.
• Documents evaluation results and progress notes as required.
• Communicates with patients, families, physicians and the care team.
