Job summary

Jobtailor in Michigan is seeking a licensed Speech-Language Pathologist to perform comprehensive evaluations of communication, cognition, voice, and swallowing, and to develop personalized plans of care with measurable goals.

The role requires CCC-SLP preferred, Michigan license, home health experience preferred; you will provide evidence-based interventions, reassess progress, document thoroughly, and collaborate with physicians and caregivers to optimize patient outcomes.

Qualifications

• Degree from an accredited Speech-Language Pathology program.
• Current Michigan Speech-Language Pathologist license in good standing.
• CCC-SLP preferred where applicable.
• Home health experience preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Excellent communication, interpersonal, and patient education skills.
• Ability to work independently while remaining connected to the interdisciplinary care team.
• Reliable transportation, valid driver's license, and current auto insurance.
• Familiarity with Medicare/CMS documentation preferred.

Responsibilities

• Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs.
• Develop individualized plans of care and measurable treatment goals.
• Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders.
• Assess and treat dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders.
• Reassess patient progress and modify treatment plans as clinically appropriate.
• Provide skilled speech therapy services in accordance with physician orders and CMS CoPs.
• Educate patients and caregivers on communication strategies, swallowing precautions, cognitive techniques, adaptive tools, and Home Exercise Programs (HEPs).
• Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements.
• Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes.
