Qualifications

• Must be currently licensed in the state of Florida.
• Minimum 2 years of experience in home health.
• Level 2 Background screening required.

Responsibilities

• Implements and provides Speech-Language Pathology services as ordered by physician.
• Develops and revises the plan of care; participates in patient care planning.
• Completes assessments, progress notes, and reports per agency timeline.
• Engages with patients and families to support communication goals.

Qualifications of Home Health Speech Language Pathologist (SLP)

• Currently licensed in the state(s) in which practicing
• At least 2 year or more of experience in home care

Responsibilities and Duties of Home Health Speech Language Pathologist (SLP)

• Improves or maximizes the communication of the patient.
• Returns the individual to optimum and productive living within the patient's capabilities.
• Periodically participates with all other health care personnel in patient care planning.
• Provides full range Speech-Language Pathology Services as ordered by physician.
• Takes initial history and makes initial evaluation.
• Performs all skilled procedures.
• Consults with physicians regarding change of treatment.
• Writes reports to physicians regarding patient's progress.
• Assists the physician in evaluating level of function.
• Helps develop the plan of care and revises as necessary.
• Prepares clinical and progress notes
• Advises and consults with the family and other Agency personnel.
• Participates in in-service programs.
• Completes and submits assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency defined time frames
