Job summary

CenterWell Home Health in Texas is seeking a Home Health Speech Language Pathologist. You will evaluate and provide speech-language pathology services to patients in their homes or facilities, contribute to Plan of Treatment, and use professional judgment to assess and treat disorders of speech, voice, language, hearing and swallowing.

You must have at least six months of relevant experience, a current license and CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects
• Observe, record and report changes in patient condition and response to treatment
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Maintain and submit documentation and reports as required by policy
• Participate in care coordination activities and discharge planning
