Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver home-based evaluations, treatment plans and ongoing care to patients in their homes or facilities. You will participate in Plan of Treatment development, supervise and guide care, and document progress per policy.

The role requires ASHA CCC-SLP or CCC-A, current license and CPR, plus at least six months of experience as a speech-language pathologist.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist/speech language pathologist.
• Current and unrestricted license with CPR certification.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Observe, record and report changes in condition to supervisor/physician.
• Provide instruction to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and progress reports per policy.
