Job summary

Starlight Home Health is seeking a compassionate Speech-Language Pathologist (SLP) to provide therapy services to patients in their homes. This role supports patients’ safety, communication, and quality of life with independent work in a home health setting.

The ideal candidate holds a Colorado SLP license, a graduate degree in Speech-Language Pathology, CPR/BLS, and a willingness to travel to patients’ homes. Flexible scheduling and a supportive team are offered.

Qualifications

Current Colorado Speech-Language Pathologist license.
Graduate degree in Speech-Language Pathology from an accredited program.
Current CPR/BLS certification.
Valid driver's license, reliable transportation, and willingness to travel to patients' homes.
Strong communication and documentation skills.
Home health experience preferred, but not required.

Responsibilities

Complete speech therapy evaluations and reassessments.
Develop individualized plans of care and treatment goals.
Provide treatment for speech, language, cognitive-communication, and swallowing disorders.
Provide dysphagia assessment, treatment, and education as appropriate.
Educate patients, families, and caregivers.
Communicate changes in condition and recommendations to providers and the interdisciplinary team.
Complete accurate and timely home health documentation.
Coordinate care with nursing, PT, OT, providers, and other members of the care team.
