Classic Home Care, a nurse-owned and operated skilled home health agency, seeks a Speech Language Pathologist for home health care in Southeast Michigan. You will provide speech pathology services, assess patient needs, and collaborate with a dedicated care team in patients’ homes.

Qualified candidates hold an active Michigan SLP license, have reliable transport, and may have prior home health experience. The role offers a flexible schedule with visits and travel to surrounding areas.

Qualifications

• Active Michigan Speech Therapy license
• Reliable transportation required
• Valid driver's license and auto insurance in your name
• Home health care experience preferred

Responsibilities

• Provides speech pathology services per physician’s plan of treatment.
• Assists in assessing patient function and plan of care with appropriate tests for speech, hearing and language disorders.
• Records and reports patient progress and any changes to physician or care team.
• Educates patient, family and health team to support patient independence.
• Recommends hearing tests by audiologist when necessary; checks hearing aides.
