Job summary

Classic Home Care is seeking a compassionate Speech Language Pathologist to provide home health services across Southeast Michigan. You will work with patients referred by a care manager, perform assessments, develop and adjust plans of care, and educate families on communication strategies.

The ideal candidate holds an active Michigan SLP license, reliable transportation, and is comfortable traveling to patients' homes.

Qualifications

• Active Michigan Speech Therapy license is required.
• Reliable transportation is required.
• Home health care experience is preferred.

Responsibilities

• Provides speech pathology services per physician's plan of treatment.
• Assesses patient functioning and contributes to plan of care development.
• Documents patient response to treatment and updates clinicians.
