Job summary

Premier-Systems in Indianapolis, IN seeks a Speech Pathologist/Audiologist to provide skilled home-based services on a part-time salaried basis or pay-per-visit option. You will evaluate, treat, and document patients' speech/hearing needs in their homes, coordinating with physicians and the care team.

Responsibilities include developing treatment plans, performing assessments, educating families, and ensuring timely charting and seamless patient care across visits.

Qualifications

• Master’s Degree in Speech-Language Pathology.
• Bachelor Degree in Speech and Hearing Therapy preferred.
• Licensed in Indiana to practice speech language pathology.
• Preferred to be a member of ASHA.
• Criminal background check required.
• Current CPR certification required.

Responsibilities

• Evaluate the patient's level of functioning and recommend mechanisms to enhance hearing.
• Provide initial and ongoing comprehensive assessments including OASIS.
• Establish a Speech Therapy treatment plan in consultation with the physician.
• Develop and implement the interdisciplinary patient care plan for Speech Therapy.
• Communicate with the physician and health team and adjust orders as needed.
• Instruct and counsel family and patient in the speech therapy program at home.
• Record evaluation data, treatments, and responses on a timely basis.
• Record and report to the physician patient status and changes in condition.
• Coordinate scheduling and discharge planning related to Speech Therapy.
