Blue Delta Tech in Indianapolis, IN seeks a part-time Speech Pathologist/Audiologist to provide home-based services. The role supports patients in their homes under physician orders, performing assessments and developing treatment plans with physicians, and coordinating closely with nursing staff.

Responsibilities include ongoing patient evaluation, family education, and ensuring accurate documentation while facilitating inter-disciplinary care and discharge planning as needed.

Qualifications

• Master’s Degree in Speech-Language Pathology required or pursuing licensure.
• Licensed in Indiana to practice speech language pathology.
• Current CPR certification and criminal background check.

Responsibilities

• Provide skilled speech pathology service in patients' homes on an intermittent basis.
• Evaluate patient functioning and recommend mechanisms to enhance hearing ability.
• Perform OASIS assessments and develop a treatment plan with physician approval.
• Care plan development with interdisciplinary team and communicate changes.
• Educate and supervise family on speech therapy program at home.
• Schedule visits and coordinate with Director of Nursing weekly.
