Responsibilities

• Conduct comprehensive evaluations of residents to assess speech, language, and swallowing disorders.
• Develop, implement, and adjust individualized treatment plans based on resident needs.
• Provide speech therapy treatments and interventions to residents.
• Prepare and maintain accurate progress notes, recertification documentation, and discharge summaries.
• Collaborate with interdisciplinary team members to ensure coordinated care.
• Complete documentation in compliance with regulatory and organizational standards.
• Discharge residents from therapy when goals are achieved or as appropriate.
