Job summary

Freedom at Home is hiring a Speech-Language Pathologist (SLP) for home health on a per diem basis. You will evaluate and treat patients in their homes, develop individualized plans, and educate families on communication and swallowing strategies while coordinating with a multidisciplinary team.

You’ll work with flexible scheduling, competitive per-visit pay, and paid mileage, all supported by a leadership team dedicated to your professional growth and high-quality patient care.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license in the state of practice.
• CCC-SLP preferred, if applicable.
• Current CPR certification.
• Home Health experience is preferred but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide skilled speech therapy services in patients' homes.
• Educate patients and caregivers on communication strategies, swallowing safety, cognitive exercises, and home programs.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Complete accurate, timely clinical documentation and participate in case conferences and quality improvement initiatives.
• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.

Education

Master's or Doctoral degree in Speech-Language Pathology

Driver's license

Current Speech-Language Pathologist license

CCC-SLP preferred
