Job summary

Home Health Care, Inc. seeks a Speech-Language Pathologist (SLP) to deliver expert evaluations and therapies to patients in their homes.

This full-time role emphasizes speech, language, cognition, and swallowing, driving meaningful functional outcomes for individuals and families. You will join an interdisciplinary team at a locally owned agency serving a 21-county area, with licensure reimbursement, MedBridge access for CE, and comprehensive benefits.

Qualifications

• Passion for helping patients regain independence and confidence.
• Excellent communication and clinical reasoning skills.
• Home health experience welcomed but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive speech, language, cognitive, and swallowing evaluations in patients' homes.
• Develop individualized treatment plans focused on meaningful functional outcomes.
• Provide evidence-based interventions for communication, cognition, and dysphagia.
• Educate patients, caregivers, and family members to promote long-term success.
• Collaborate closely with physicians, nurses, therapists, and the interdisciplinary care team.
• Complete timely and accurate documentation using our electronic medical record.
