Job summary

Freedom at Home is seeking a Speech-Language Pathologist (SLP) to join our home health team on a per diem basis. We offer excellent pay, paid mileage, and flexible scheduling to support clinicians who deliver compassionate, patient‑centered care in patients’ homes.

The ideal candidate will have an SLP license in the state, CCC‑SLP is preferred, and CPR certification. Home health experience is a plus, with a valid driver’s license and reliable transportation to serve patients in the community.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• Current Speech-Language Pathologist license in the state of practice.
• Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.
• Current CPR certification.
• Home Health experience is preferred but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide skilled speech therapy services in patients' homes.
• Educate patients and caregivers on communication strategies, swallowing safety, cognitive exercises, and home programs.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Complete accurate, timely clinical documentation and participate in case conferences and quality improvement initiatives.
• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.
