Job summary

Elara Caring seeks a Speech-Language Pathologist to join our home health team in Indiana, delivering high-quality care in patients' homes. You’ll assess and treat communication and swallowing disorders, develop plans of care, and collaborate with physicians and caregivers for safe, effective therapy.

Must hold a Master’s in Language Pathology, an active SLP license, and be willing to travel up to 50% with a valid driver’s license.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.
• Willing to travel within branch/office coverage area; travel up to 50%.
• Valid driver’s license and auto insurance as per state laws.

Responsibilities

• Promotes Elara Caring’s philosophy and policies to ensure quality of care.
• Evaluates patients and develops a plan of care per findings.
• Admits patients and assesses therapy needs per regulatory guidelines; performs initial and follow-up evaluations.
• Initiates safety plans with patient, family, and community resources.
• Communicates findings and changes to supervising physician and team.
