Job summary

UT Southwestern Medical Center seeks a dedicated Social Worker to provide coordinated care in the home. You will create patient-centered action plans to promote education, self-management, and adherence to the PCP plan of care, facilitating timely access to healthcare delivery.

Requirements include a Master’s degree in social work and a current Texas license; 1 year of health-care experience with homebound patients is preferred. This is a Per Diem - PRN role located in Dallas, TX.

Qualifications

• Master's degree in social work required.
• Current license by the Texas State Board of Social Work Examiners required.
• 1 year of experience in a health care setting preferred, particularly with homebound patients.

Responsibilities

• Ensures delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs.
• Participates in development and update of Plan of Treatment.
• Evaluates patient and home environment to determine social and psychological factors related to patient's health problems.
• Prepares and submits to agency psychosocial histories, clinical and progress notes. Observes and reports on patient/family reaction to treatment.
• Counsels patient and/or family in understanding, accepting, and adjusting to patient's health problems and emotional care.
• Identifies, informs, and refers patient/family to available community resources.
• Coordinates discharge planning.
• Interprets to the community the role of social work in the Agency's programs.
• Participates in planning, development, and delivery of staff in-service education programs.
• Participates in Continuing Quality Improvement Program. Monitors adherence to policies, procedures, and protocols.
