Job summary

Elite Care Home Health & Hospice seeks a Licensed Master Social Worker (LMSW) to join its interdisciplinary team in the Dallas, TX area. The LMSW will conduct psychosocial assessments, provide counseling, coordinate resources, and advocate for patients and families across Home Health and Hospice services.

Requires an MSW and Texas LMSW license, with at least 1 year in healthcare or social services preferred. Part-time, PRN with flexible scheduling and strong patient-centered care emphasis.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited college or university.
• Current Texas LMSW license in good standing.
• Minimum one year of healthcare, Home Health, Hospice, hospital, or social services experience preferred.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Develop individualized plans of care based on patient needs and goals.
• Provide emotional support, counseling, and crisis intervention.
• Assist with advance care planning and healthcare decisions.
• Coordinate community resources and referrals.
• Educate patients and caregivers about services and benefits.
• Participate in IDG meetings and collaborate with healthcare professionals.
• Support families with grief, loss, and caregiver stress.
• Maintain compliant clinical documentation.
• Advocate for patient rights and patient-centered care.
