You will conduct psychosocial assessments, connect clients with community resources, advocate for benefits, and collaborate with healthcare professionals to coordinate comprehensive, person-centered care.

Qualificações

• MSW degree and CA social work license (LCSW preferred).
• Experience with geriatric populations or home health services.
• Strong communication, empathy, and resourcefulness.

Role Overview:

As a Home Health Social Worker, you will help seniors access essential services, provide emotional support, and guide families through healthcare, financial, and social resource navigation. Your involvement ensures a holistic approach to elderly care, improving quality of life and peace of mind.
