Job summary

Cape Regional Home Health Care Managed by BAYADA is seeking an experienced Speech Language Pathologist (SLP) for home health visits in Cape May County, NJ. This role is available as full-time or part-time, with a $5,000 sign-on bonus for full-time.

The SLP will evaluate, diagnose, and treat adults and geriatric patients in their homes, develop and adjust treatment plans, document care, and collaborate with a multidisciplinary team to promote safe, independent living.

Qualifications

• Current NJ license as a Speech Language Pathologist (SLP).
• ASHA CCC or equivalent credential or eligibility for certification.
• Experience with electronic medical records (EMR) preferred.
• Solid computer skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and treat speech, language, communication or swallowing difficulties.
• Develop a treatment plan based on evaluation, physician orders and goals.
• Ongoing assessment and revision of the care plan with the multidisciplinary team.
• Educate patients and family members in rehabilitative care to promote health and independence.
• Accurately document observations and interventions using an EMR tool.
