VitalCaring is hiring a Speech Language Pathologist (SLP) for home health in Oklahoma City. You will assess and treat speech, language, cognition, and swallowing, creating individualized plans that empower patients to communicate and live more independently at home.

You’ll work autonomously with clinical leadership behind you, delivering one-on-one care that centers on meaningful outcomes and patient safety. Travel to patient homes is expected, with opportunities for professional growth.

Qualifikationen

• Master’s or Doctoral degree in Speech-Language Pathology.
• Current SLP license in the state of practice.
• Minimum 1 year clinical experience (home health preferred).
• CCC‑SLP certification preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient education skills.
• Comfortable in field-based role with travel between patient homes.
• Current CPR certification.
• Valid driver’s license and transportation.

Aufgaben

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function.
• Develop and implement individualized plans of care to improve communication, cognition, and safety.
• Provide therapeutic interventions using evidence-based techniques.
• Assess home environment and recommend adaptive equipment or communication aids.
• Educate patients and caregivers on exercises and safety techniques.
• Monitor progress and adjust treatment plans as needed.
• Communicate proactively with physicians and care team.
• Document all care accurately in the EMR.
• Collaborate with nurses and therapists for coordinated care.
• Participate in case conferences and discharge planning.
