VitalCaring Group in the United States is seeking a Home Health Speech-Language Pathologist to evaluate and treat patients with speech, language, cognitive, and swallowing disorders in the home.

This role emphasizes autonomy, travel between patient homes, and collaboration with nurses and therapists to deliver evidence-based care and measurable outcomes for daily living and communication.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology required.
• Current SLP license in the state of practice.
• CCC-SLP certification preferred.
• Strong communication and patient education skills are essential.

Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function.
• Develop and implement individualized plans of care to improve communication and safety.
• Provide evidence-based therapy interventions using data-driven approaches.
• Assess the home environment and recommend adaptive equipment or communication aids.
• Educate patients and caregivers on exercises, compensatory strategies, and safety techniques.
• Monitor patient progress, reassess as needed, and adjust treatment plans accordingly.
• Identify changes in condition and communicate proactively with physicians and the care team.
• Document all care accurately and timely within the EMR.
• Collaborate with nurses, therapists, and interdisciplinary teams for coordinated care.
• Participate in case conferences, discharge planning, and ongoing care coordination.
