Job summary

Humana Inc. through CenterWell Home Health seeks a licensed Speech-Language Pathologist to deliver in-home care. You will evaluate, direct and provide services to patients, participate in Plan of Treatment and Care, and document progress and care coordination.

You'll work under the Clinical Manager, with requirements including ASHA CCC or in Audiology, current license and CPR, and a valid driver’s license. Pay range $85,400–$117,500 per year.

Qualifications

• CCC in Speech-Language Pathology or Audiology per ASHA.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Preferred: Home Health experience.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapy applications.
• Observe, record and report changes in patient condition to supervisor or physician.
• Provide instruction and training to patients in use of alternative communication systems.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company.
• Prepare and submit written reports and care coordination activities.
• Participate in care coordination and discharge planning.
