Job summary

Humana Inc. and its CenterWell Home Health division seek a Speech-Language Pathologist to provide in-home evaluations and treatments for speech, voice, language, and swallowing disorders.

You will develop and review the Plan of Treatment, educate patients and families, and coordinate care across the team. Applicants should hold ASHA CCC certification or licensure, possess a valid license and CPR, and have a valid driver’s license.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA)
• Minimum of six months experience as a speech therapist / speech language pathologist
• Home Health experience is a plus
• Current and unrestricted license
• Current CPR certification
• Good organizational and communication skills
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in the home or facility
• Develop and review the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and therapeutic treatments
• Observe, record and report changes in patient condition to supervisor/physician
• Provide instruction in alternative communication systems when appropriate
• Counsel patients, families and healthcare staff
• Maintain and submit documentation
• Coordinate care and discharge planning
• Adhere to confidentiality and professional conduct standards
• Participate in staff development and continuing education
