Job summary

Boost Home Healthcare in Joliet, IL is seeking a licensed Speech-Language Pathologist to deliver therapy to patients in their homes under physician orders and plan of care.

You will assess, plan, implement and reevaluate treatment, coordinate with physicians, supervise home health aides, and contribute to discharge planning.

Flexible scheduling and potential work-from-home options are available, with a focus on helping patients maintain independence in the comfort of their own homes.

Qualifications

• Master’s or Doctoral degree in Speech Language Pathology required.
• Current state license to practice as a Speech Language Pathologist required.
• Minimum of one year in an acute care setting preferred.

Responsibilities

• Gather background data, review referrals, select and modify evaluation tools, and evaluate patients.
• Reevaluate progress during treatment and modify necessary adaptive equipment to improve independence.
• Provide full range of speech language pathology services as ordered by physician.
• Consult with physicians regarding changes in treatment.
• Supervise home health aide as appropriate per state guidelines.
• Evaluate outcomes and discharge planning.
