Job summary

Life Care Home Health is hiring a Speech-Language Pathologist to provide coordinated care to clients of all ages in home settings across Florida. You will plan, implement, and evaluate care plans, applying therapies for speech, language, swallowing, and cognitive disorders.

You will document plans, progress notes, and communications with families and other health professionals while maintaining compliance with infection control and state licensure requirements.

Qualifications

• Master’s degree or doctorate in Speech-Language Pathology from an accredited college or university.
• Current state professional license to practice as an SLP in applicable states.
• Current CPR certificate and valid driver’s license

Responsibilities

• Perform clinical assessments and determine client needs and plan of care.
• Document and revise Plan of Care, progress notes, and orders.
• Analyze client needs and provide care as ordered by physician.
• Apply infection control and universal precautions in client care activities.
• Communicate effectively with clients, families, and health care team; participate in case conferences.
• Monitor cases for third-party payer requirements.
• Complete documentation timely to ensure policy compliance.
• Engage in in-service programs to maintain competency and pursue continuing education.
• Meet mandatory continuing education requirements of the agency or licensing board.
