Concierge Home Care is seeking a Home Health Speech Therapist (SLP) for Jacksonville, FL. Earn $95K+ annually with a flexible schedule and autonomy to manage your own caseload in patients' homes.

Qualifications

• Active Florida Speech Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred, but not required.

Responsibilities

• Provide speech therapy services to patients in their homes on an intermittent basis.
• Assess speech, language, and swallowing conditions.
• Develop personalized, goal-oriented treatment plans with patients and caregivers.
• Deliver therapy per physician orders and care plans under supervision of Clinical Services leadership.
• Educate patients and caregivers on strategies to improve communication and swallowing function.
