Job summary

Ohana Pacific Health in Hawaii seeks a Speech Language Pathologist to develop and implement a home-based rehabilitation program for patients, screen and treat disorders of speech, language, cognition and swallowing, and serve as case manager.

Qualifications

• Master's degree in Speech/Language Pathology from ASHA-accredited program.
• Hawaii state license to practice as a Speech-Language Pathologist.
• Current CPR/AHA Basic Life Support certification.
• 2-step TB clearance and health screening on file.

Responsibilities

• Develop and implement a progressive home-based rehabilitation program.
• Screen, diagnose and treat communication, cognition, and swallowing disorders.
• Serve as case manager and supervise Home Health Aides when appropriate.
• Perform patient evaluations and determine factors contributing to impairments.
• Coordinate treatment plans and re-assess patients regularly.
