Qualifications

• Medicare Home Health Agency experience preferred.

Responsibilities

• Provide leadership for an interdisciplinary team and guide patient goals.
• Perform comprehensive history and physical on admission and educate patient and family about health.
• Promote collaborative communication among disciplines involved in the patient’s care.
