Job summary

Ohana Pacific Health in Honolulu is seeking a dedicated Physical Therapist to lead a progressive home health rehabilitation program, delivering patient-centered care in the home setting and serving as a case manager for diverse medical needs.

You will evaluate patients, plan and coordinate treatments, supervise Home Health Aides and PTAs as appropriate, and discharge when goals are met, ensuring regulatory compliance and high-quality outcomes.

Qualifications

• Master’s or Doctorate in Physical Therapy.
• Hawaii license to practice as a Physical Therapist.
• Current CPR (BLS) certification.
• Two years of PT experience, preferably in home health.

Responsibilities

• Evaluate assigned patients and admit those meeting criteria.
• Plan, coordinate and implement treatment; reassess progress.
• Discharge patients when goals are met or requirements end.
• Develop HHA and PTA care plans; supervise HHA/PTA staff.
• Handle case management and coordinate referrals.
