Job summary

Hanford Home Health is seeking a Per Diem Home Health Skilled Nurse (RN) to care for EEOICPA beneficiaries and other medically complex patients in the home. The role offers a flexible schedule with shifts posted in advance, requiring commitment to accepted assignments.

You will perform RN-level assessments, administer medications, monitor patient conditions, and document outcomes per the Plan of Care. Idaho RN license, CPR/BLS, and driving eligibility are essential.

Qualifications

• Active Idaho State RN license in good standing.
• Graduate of an accredited School of Nursing.
• Minimum of one year of nursing experience; home health preferred.
• Current CPR/BLS certification for healthcare professionals.
• Idaho Food Worker’s Card or ability to obtain upon hire.
• Valid driver’s license and reliable transportation.

Responsibilities

Home Health Skilled Nurse (RN) – Per Diem

Hanford Home Health is hiring Per Diem Home Health Skilled Nurses (RN) to provide skilled nursing care to EEOICPA benefits recipients and other medically complex patients. As a Per Diem skilled nurse, you will work on an as-needed basis, providing expert care to patients in their homes. We are seeking dedicated and compassionate nurses who share our mission to deliver exceptional patient care.

Schedule & Shift Structure

As a Per Diem skilled nurse, you will work on a flexible schedule, with shifts posted one (1) month or more in advance when applicable. You will be expected to complete all assigned shifts accepted, subject to agency attendance and call-out policies. Per diem status does not guarantee hours, ongoing assignments, or priority scheduling.

Key Responsibilities

• Provide skilled nursing care in accordance with the authorizing practitioner’s orders and the individualized Plan of Care
• Perform nursing assessments and ongoing monitoring within RN scope of practice
• Administer medications and treatments; monitor for side effects and changes in patient condition
• Provide skilled interventions as ordered (including wound care and other therapeutic interventions when within scope and ordered)
• Measure and document vital signs, intake/output as applicable, pain, weight, and patient responses per the Plan of Care

Required Qualifications

• Active Idaho State RN license in good standing
• Graduate of an accredited School of Nursing
• Minimum of one (1) year of nursing experience; home health experience preferred
• Current CPR/BLS certification for healthcare professionals
• Idaho State Food Worker’s Card (or ability to obtain upon hire)
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to pass background check and drug screening
• Strong clinical judgment, documentation, and communication skills

Preferred Qualifications

• Experience in home health, hospice, or geriatric care
• Prior experience with electronic documentation systems

Pay & Benefits

• Pay Rate: $55.00 per hour (base rate) with the possibility of shift differential pay
• Overtime is paid in accordance with Idaho State and federal law
• Travel within the Hayden and surrounding areas; mileage reimbursed at $0.725 per mile
• Paid Sick Leave provided in accordance with Idaho State law

Conditions Of Employment

• Employment contingent upon meeting agency health, immunization, and regulatory requirements
• Completion of required employment eligibility verification

• Idaho State background check and disclosure required at hire and every two (2) years and as needed
